Mobilize & Evolve is a non profit that helps individuals with Intellectual and Developmental Disabilities (IDD) and Autism participate more fully in their communities by connecting them with resources to cover costs associated with transportation and moving.
Since 1998, we have worked to help people with IDD and Autism enjoy the full benefits of their communities by procuring better access to employment opportunities, leisure activities, religious institutions, and social outlets.
Mobilize & Evolve helps people who have IDD and Autism secure funds to secure a rideshare service or hire moving help. The following is a brief outline of our referral process.
Referral Process
Mobilize & Evolve helps people who have IDD and Autism secure funds to secure a rideshare service or hire moving help. The following is a brief outline of our referral process.
- A Support Coordination Organization (SCO) must complete the referral form (Click Here) for individuals who need help procuring funds for moving homes (renting a truck, hiring movers, etc.) or securing transportation from a rideshare service (Uber/Lyft).
- We will contact the SCO within 48 hours of receiving a completed referral form.
- Once approved, resources will be released to the SCO or delivered directly to the moving company.
Eligibility
- An individual must have an SCO company involved.
- An individual must have a Consolidated, Person/Family Direct Support, Community Living or Adult Autism Waivers
- An individual may receive no more than two (2) $100 gift cards for rideshare services (Uber/Lyft) within a calendar year (Jan 1 - Dec 31).
- An individual may receive moving funds only once (1) within a calendar year (Jan1 - Dec 31).
